Kirei Kotomine has no relation with Caster...to make it clear...i'll breakdown the master and servant relationship to you...

Servant: Saber
Masters:
Shirou(Fate,UBW,HF)
Caster(UBW)
Rin(UBW)
Sakura(HF) ==>as Shadow

Servant: Lancer
Masters:
Kotomine Kirei(Fate,UBW)
Lancer only appeared a few times and eventually defeated in HF

Servant: Archer
Masters:
Rin(Fate,UBW,HF)
Caster(UBW)

Servant: Rider
Masters:
Shinji(Fate,UBW)
Sakura(HF)
In HF, Rider's true master is Sakura, Sakura only passed down rider to Shinji

Servant: Caster
Masters:
Kuzuki Souichirou(Fate,UBW,HF)
Caster killed her true master but Souichirou found him and Caster fell in love with him

Servant: Berserker
Master:
Ilya(Fate,UBW,HF)

Servant: Assassin
Masters:
Never revealed his master in Fate
Caster(UBW,HF)

Those are the original 7 servants that normally participates in the HGW
but there are other servants as well:

Servant: Gilgamesh
Master:
Kotomine Kirei(Fate,UBW,HF)

Servant: True Assassin
Master:
Matou Zouken(HF)
True Assassin is out of Assassin's body and appeared only in HF Scenario

Servant: Black Berserker

==if you're wondering why are there two masters for one scenario...it's because the original master/servant was defeated and gained by the winning master/servant...

i don't know who is the Kotomine teacher you're talking about...lol...
Kotomine is the guy Rin and Shirou met in the Church...
that's why the church's name is Kotomine Church...

He is a participant in the HGW from previous and present wars...
his father is killed during the last war so he thought to himself that he will continue to do what his father has done...

Caster's true master was never revealed...she was only taken by Souichirou and become her master...

maybe you are asking about Lancer...true, lancer and gilgamesh are both servants of kotomine...the last war never ended so kotomine still has gilgamesh and summoned lancer for the next war...

And you might be wondering why is Caster the master of Assassin...well it's not written in the rules for a servant to be master...

well, basically if you're refering to black saber and black berserker, there are just under the control of Shadow, asides from that all of the servants have their background revealed in the game...if you went throught the 3 different scenarios you'll probably know the background of all 7 plus the extra ones...

i think Geneon Ent. will try to alter the game's storyline. i don't know, i feel like they are trying to put in 3...cause based on an anime review that i read, they said that the anime will cover fate & ubw scenarios. but since the last episode introduces sakura to the war, maybe they'll try to fit everything right..peace..

I can't really tell you if Fate/Hollow Ataraxia is a sequel. It was released because of Fate/Stay Night's popularity in japan. So consider it as a side story. But the game actually took place half year after the 1st game. But it did not tell which is the true ending that the sequel followed, making it really confusing if it's a sequel. In this game Shirou is not the actual lead character.
Basically all of the Servants from previous game except True Assassin was there. New servant was introduced though, his class is Avenger.
